What is the Ageing with a Disability Program?
People receive support to participate in activities of their choice
The Ageing with a Disability Program, based in the Northern Metropolitan Region, offers support and assistance for people with a disability over the age of 30, still living with family members, to manage their current living situation and to plan and prepare for the future. Individuals must be eligible for assistance under the Intellectually Disabled Persons Services Act (1986) or the Disability Services Act (1991).
We recognise that the current situation for individuals, or families caring for an older person with a disability at home, may not be able to continue into the future and are able to help all involved with planning and preparation for change.
What can the Ageing with a Disability Program offer?
The program offers a range of options that are used to develop a specific 'package' of support to meet the needs of each individual and / or family circumstances.
Case management
A Case Manager will provide you with information and support to manage your current situation (if required) and to assist in identifying future needs and planning for these. Some examples of assistance provided are:
- Exploring alternative living arrangements for the person with a disability;
- Respite from the caring role;
- Addressing the health needs of the older carer;
- Estate and will planning;
- Links to community educational or recreational options for the person with a disability;
- Contact with carer support networks;
- Referral to additional support / assessment / counseling services as required;
- Social contacts and support for the person with a disability; and
- Skill development opportunities for the person with a disability.
Accommodation transition support
A six bed transition accommodation house is available to assist individuals and families 'make the break' and prepare for the person with a disability moving to alternative accommodation. The house is also able to provide respite for the carer during the transition period. Regular planned stays are a feature of this aspect of the program and to assist with the change, carers are able to stay with the person with a disability in the initial phase if required.
Community Support
The 'Community Support' component of the service specialises in providing support to people with a disability who are over 55 and either still living at home or in supported accommodation. Support is provided to enable people to access a variety of recreation, leisure, social, education and vocational activities while gaining the confidence, skills and/or resources to continue independently. Emphasis is placed on personal choice so that the support provided can be specifically tailored to maximise abilities and create and maintain community networks within the local area.
