The Commonwealth Carelink Centre (CCC) is a Commonwealth Government initiative. The CCC has been set up to provide links between health and community services, providing individuals, carers, services providers, general practitioners and other health professionals with a single point of access for information about and referral to a range of health and community care services. The service is offered nation-wide, with staff located regionally.
