Care packages provide case management and services that support frail older people to maintain their independence and remain living at home and active in the community. Villa Maria assists with Community Aged Care Packages (CACP), Extended Aged Care at Home Packages (EACH), Extended Aged Care at Home Dementia (EACH D) and Assistance with Care and Housing for the Aged (ACHA).

To be eligible for the CACP, EACH or EACH D package, a current Aged Care Assessment Service (ACAS) is required. This will determine whether you require high-level care or low level residential care and if you would prefer to stay living at home. The ACAS Assessment is free.

Villa Maria provides Case Managers to assist carers and care recipients to plan the care and support they require and co-ordinates the provision of services on their behalf.
 

CACP - Community Aged Care Packages (available in all regions)  

Community Aged Care Packages (CACP) (low level care) are individually planned and coordinated packages of care services tailored to assist a frail older person to remain living in their own home. CACP is available in all regions.
 

EACH - Extended Aged Care At Home (available in Eastern and Loddon Mallee regions)

Extended Aged Care at Home (EACH) Packages (high level care) are planned and coordinated packages of community care services to assist a person to remain living at home when normally they would need to receive a high level of care within an aged care (nursing home) facility.
 

EACH D - Extended Aged Care at Home Dementia (available in Eastern and Hume regions)

Extended Aged Care at Home Dementia (EACH D) Packages (high level care) are planned and coordinated packages of community care services to assist a person who has dementia and/or behaviours of concern and wishes to remain living at home when normally they would need to receive a high level of care within an aged care (nursing home) facility.

ACHA - Assistance with Care and Housing for the Aged (available in the Eastern region)

Assistance with Care and Housing for the Aged (ACHA) offers assistance to frail, disadvantaged older people who are renting or who are at risk of homelessness, in order to meet their accommodation and support needs.

ACHA is available to people who are living in the Cities of Knox and Maroondah and the Shire of Yarra Ranges who:

• Are aged over 50 years
• Receive a low income and not own a house
• Live in rented and/or insecure housing
• Are at risk of homelessness
